The Restored Bodyworker Massage Therapy

RBW Membership Terms & Conditions

Membership includes: 1 60 or 90 minute Massage each month.  Additional massages during the same month will be paid at time of service for the membership rate.

Membership fee of ___________ is charge on the 1st day of each month to a credit card on file with RBW. Membership fees are non-refundable. Unused services expire 30 days after purchase date or upon termination of this membership agreement.

This agreement begins on ___/____ (month/year) for 1 year and ends on ____/_____(month/year)

All services are by appointment only.  Late Arrivals and Cancelation polices are still apply to membership. 

Late Arrivals:  the schedule appointment will end on time.  At the discretion of the of the massage therapist, and if time allows, the entire appointment may still be honored. Habitual lateness will not be granted full massage session regardless of time.

Cancellation Policy:  24 hours notice of cancellation required. Appointment may be rescheduled before the end of the current month without penalty.

If member is unable to use service in that month, member may transfer the session to someone else residing in their immediate household, regardless of marital status.

Clients may suspend this agreement for up to 3 months if they are unable to receive a massage due to a medical reason.  If the medical issue is not resolved after the 3 months, this membership agreement will be terminated.  A doctor’s note may be required at the discretion of RBW to continue the membership agreement.

Clients may terminate this membership agreement at anytime AFTER the first 3 months by submitting in writing a cancelation request to The Restored Bodyworker.  Cancelation requests must be received NO LATER than the 15th of the month to avoid any additional charges for the next month.

The Restored Bodyworker will terminate this membership agreement if unable to process the monthly membership fee within the applicable month.  The client will be responsible for providing an alternate form of payment to remain on the currant membership contract.  If client fails to provide a new form of payment or cancels within the initial 3 month period, client will be responsible for the remaining 3 month initial period payments as well as, any bank/credit card processing incurred charges to RBW.

The Restored Bodyworker reserves the right to refuse service to any client who has acted in an inappropriate manner or who was not forthcoming about his/her medical condition that is a contraindication to receiving massage therapy when the appointment was made.  If service is refused on these grounds, the membership contract will be terminated immediately and all future appointments canceled.  Member will still be responsible for payment of remainder of the initial 3 month period.

If membership contract is terminated under terms #6, #7 or #8, the client may choose to sign a new membership contract agreement at the then current rate.  If a contract is terminated under term #9, the client will not be eligible for a new membership contract.

I authorize The Restored Bodyworker Massage Therapy to keep a credit card on file and to charge my credit card monthly for the membership fees as stated above.  By signing this authorization, I acknowledge that I have read and agree to all the terms stated above.
